The triple tiara, dove, and crossed sceptre are all common attributes of St. Gregory.

In the background is Rome's Castel Sant'Angelo. According to the Golden Legend after Gregory led a procession to pray for relief from the plague he saw an angel appear above the Tomb of Hadrian and sheath his sword as a sign that "the pestilence of this mortality was passed." Thereafter the tomb was known as Castel Sant'Angelo, the Castle of the Holy Angel.
